Elevated Health RX, LLC ("Elevated Health RX," "we," "us," or "our") operates an online telehealth platform that connects patients with a network of licensed U.S. healthcare providers for the evaluation, diagnosis, and treatment of erectile dysfunction and related men's health conditions. Our providers may prescribe compounded medications, including PDE5 inhibitors such as Sildenafil and Tadalafil, which are dispensed by FDA-regulated 503A outsourcing pharmacies and shipped directly to you.

We receive from Stripe only a tokenized representation of your payment method, the last four digits of your card, the card brand, and the transaction result.
